About our Custom Mouthguards
DE Mouthguards are proud to partner with the Swanbourne Tigers to offer our premium custom No GAP Mouthguards for all of the players and families with dental extras cover, or $150 with no insurance.

Item Codes charge to health fund will be a 013 (limited oral exam) & 071 (mouthguard)
IMPORTANT
An Individual booking is required per person. If you require two or more bookings, please continue to use the booking link to make subsequent appointments. It is also important to note that a Mouthguard Registration Form is also required per person/booking. This link will be contained within your confirmation email post booking. This form should only be completed once a booking has been made.

Gap Free Kids
If your kids are in need of their regular dental check up, we wanted to let you know that we also offer “Gap Free Kids” dental treatment for patients that belong to one of our preferred provider health fund. If your child is covered under a dental extra’s policy with either Medibank, HBF, HCF, nib or CBHF then all check ups and treatments are completely GAP Free with our practice, all year round.
If you would like to book an appointment, please book via our practice website. You can advise us on the day about the mouthguard and it can all be completed in one visit.
