Across the next few pages you will be able to provide all of the information we require to be ready to produce your custom mouthguard.

Note: Registration forms will not be accepted unless a mouthguard booking has been made prior to completing this form.

1. Mouthguard registration form required for each person. This will help expedite the booking, impression and manufacturing.
2. Our Custom Mouthguards are GAP Free (no out of pocket cost) with all Private Health Insurance Extras cover (costs apply for no insurance)
3. Mouthguards take approx. 2 weeks post impression and will be either posted (costs apply) or taken to your club.
4. Please ensure you bring your health insurance card with you on the day or have your e-card preloaded on your phone. Most funds now support e-cards that can be applied through apple pay or android
